General Questionnaires & Screenings
Strengths and Difficulties Questionnaire - Screens for problems related to anxiety, depression, attention, and prosocial behavior. Available for Parents/Guardian, Educators, and Clinicians
Level 1 Cross-Cutting Symptoms Measures - contains questions to screen for depression, anxiety, mania, irritability, and other mental health concerns. The following assessments are specific to their mental health concern and are done for further investigation.
Patient Health Questionnaire (PHQ) - questionnaires that screen for depression and anxiety, the longer version also screening for mood problems, eating disorders and somatoform disorders. (Click on the link to see the Instruction Manual of the PHQ)
